According to the Shiva Purana's Rudraksha Mahatmya, on a Thursday during Siddhi Yoga, when rain falls from the sky and a flower of the Rudraksha closes instantly, an Ek Mukhi Rudraksha can be formed.
As per folklore, only three such Rudrakshas grow on a tree in twelve years.
Out of these, one goes to the heavens, another to the underworld, and the third remains on earth. It is believed that this one can only be obtained on Shivaratri, full moon days, or other special occasions.
